This questionnaire helps your clinician prepare for the consultation and ensures they have a thorough picture before our appointment.

Please complete all sections as fully as possible. Set aside around 20–30 minutes. The more detail you can provide, the better.

Fields with an * mean they are required fields

At the end of the survey, you will be sent several emails. One of the emails will be a medical request for you to forward to your vet. We cannot request the medical history from your vet directly, this must come from you.

If you do not see these emails, please check your spam folder. If emails land there, is critical that you mark them as "not spam" and "trust this sender".
